Remaining at the forefront of community health requires a significant investment of time, commitment and financial resources. We are extremely grateful to our current funders.
However, like all charities, we proactively raise financial resources and form other partnerships in order to respond to growing and complex needs in our community. It is crucial for us to raise these additional funds in order to address the sharp rise in chronic conditions like diabetes, maintain safe neighbourhoods, support our aging population, combat poverty, provide community-based health care and promote life-saving health education.

Why invest in the Scarborough Centre for Healthy Communities?
The SCHC community health model saves lives and money.
Our community health organization model is life-saving and cost-effective. We do not simply treat “sickness”, we promote health. Our model focuses on prevention, early intervention, health promotion and community-based health care. This holistic approach shortens and decreases hospital stays, reduces emergency room visits, abates the institutionalization of seniors and prevents “sickness”.
The SCHC community health model is integrated.
As one of a few integrated community health organizations in Toronto, we are able to address the holistic aspects of health including physical, mental, social, financial and environmental.
For example, when our social support staff uncovered that 27% of our food bank users had at least one family member with Type II diabetes, we immediately consulted with our physicians and diabetes educators. Together, our team devised a strategy that included fundraising for new equipment to store fresh produce and connecting food bank users to diabetes education workshops and screening.
This swift intervention was made possible because of our integrated model. Our uniquely diverse range of community health professionals enables us to address both primary health and social support issues in an integrated and cost-effective manner.

At SCHC, our community members are at the centre of everything we do. We understand that many of them have challenges accessing community health care and services. That is why we take the time to work in partnership with our community members, partner agencies, hospitals and others to ensure that our programs and opportunities are responsive and welcome in the communities we serve.
