FUNDAMENTALS OF PALLIATIVE CARE

(Please note only Visa or MasterCard accepted) This 6 week (3 hour classes) standardized curriculum is based on A Model to Guide Hospice Palliative Care, 2002, specifically the Domains of Issues Associated with Illness and Bereavement is funded by the Central East LHIN. The “FUNDAMENTALS” course is recommended for: ALL Health Care Providers and Hospice Palliative Care Volunteers. There is a $50.00 fee to cover the cost of your resource guide. This is non-refundable or transferable. |

“The Fundamentals of Hospice Palliative Care” is a prerequisite course for CAPCE (Comprehensive Advanced Palliative Care Education - for RPNs, RNs, NPs, and other advanced care clinicians).
***Please note*** RPNs, RNs, NPs, and other advanced care clinicians wishing to take the CAPCE course are required to complete, free of charge, a two-week Enhanced Fundamentals course after the 6-week basic Fundamentals course.

The Comprehensive Advanced Palliative Care Education (CAPCE) Program combines the “art” and “science” of Hospice Palliative Care (HPC) for nurses (RPNs, RNs, NPs, and other advanced care clinicians). The program embeds best practice standards and aligns with the Model to Guide Hospice Palliative Care. CAPCE focuses on the development and role of the nurse as a HPC resource for the inter-disciplinary team in long-term care homes, agencies, hospitals and communities.
***Please note*** RPNs, RNs, NPs, and other advanced care clinicians wishing to take the CAPCE course are required to complete, free of charge, a two-week Enhanced Fundamentals course after the 6-week basic Fundamentals course.
CAPCE can be completed in a minimum of 20 weeks. Fundamentals Enhanced is provided within the first 2 weeks of the CAPCE course.
The components of the program include: self- directed reading and reflection, 6 e-learning modules, 3 in-person sessions (case based learning), 8 peer to peer exchanges (learning debrief), 2 practical application assignments and 9 individual or small group coaching sessions.
CAPCE Selection Criteria: a minimum of 1 year hospice palliative care nursing experience, have access to an internet-enabled computer, have knowledge of basic computer programs, and have completed the Fundamentals Core and Enhanced Hospice Palliative Care Programs. To register, click HERE.
