About this Program

The Assisted Living program is designed to help senior residents remain living independently and safely in their homes for as long as possible. Our team of professional Personal Support Workers are onsite 7 days a week providing services to our clients. Using an evidence-based assessment tool, information will be gathered from the client, their family and caregivers to create an individualized care plan created based on their needs and provincial guidelines.
The care plan may include; safety checks, medication reminders, personal care and essential light housekeeping.

The Assisted Living program is based out of two (2) specified Toronto Community Housing buildings. All clients must be residents of these buildings in order to receive services from the program. Scarborough Centre for Healthy Communities is not the landlord and does not provide rental services. Individuals interested in applying to Toronto Community Housing may do so through Access to Housing at toronto.ca/accesstohousing.
